Physician's Statement/Immunization Record
Note: Receiving the completed Enrollment Agreement and Registration Fee ensures your child a place in our program. Other registration forms should be completed and returned as soon as possible. Acceptance of children will not be discriminated against because because of race, creed, or religion.
